Terms of Use
Last updated: 15 August 2026
By using denialdesk.docsanddeadlines.com ("this site"), you agree to the terms below. If you do not agree, please stop using the site.
What this site is
Denial Desk publishes general reference material about health insurance denials, claim appeals, medical bills, and the federal and state rules that govern them in the United States. It explains how to read a denial letter or an explanation of benefits, what deadlines the published rules set, which documents you can request, and where each of those rules is written down.
It is a reference, not a service. No page here reviews your particular letter, evaluates the strength of your claim, prepares or files anything on your behalf, or communicates with your insurer, employer, plan administrator, or provider. Reading this site does not put anyone to work on your case.
No professional advice
Content here is general information, not professional advice.
The author is not a lawyer, physician, licensed insurance producer, tax professional, or certified patient advocate, and nothing on this site is offered as legal, medical, tax, or insurance advice. Reading this site, or contacting the author, creates no attorney-client, physician-patient, fiduciary, or other professional relationship.
Health coverage is governed by your specific plan documents and by the law that applies to your plan and your state — and the same denial can carry different rights depending on whether your plan is self-funded or fully insured, ERISA-governed or state-regulated, private, Medicare, or Medicaid. General material cannot resolve that for you. For anything binding, consult your plan administrator, your state insurance department or Medicaid agency, Medicare, or a qualified professional. Decisions about your medical care belong with your clinician, never with a web page about billing codes.
Accuracy and currency
Guides are researched against official sources and dated on publication. Even so:
- Deadlines, dollar thresholds, forms, and appeal procedures change — sometimes mid-year, and sometimes only for certain plan types — and your own plan document may impose terms that differ from the general rule described here.
- A guide reflects the situation at the date shown on it, not necessarily today.
- Errors happen.
No warranty is given that content is accurate, complete, or current. If you find something wrong, please report it via the contact page — corrections are made promptly.
Limitation of liability
To the fullest extent permitted by law, the site owner is not liable for any loss, cost, delay, or damage arising from your use of, or reliance on, anything published here — including missed deadlines, rejected applications, or expenses incurred by following a guide. You use this information at your own risk.
